Who can refer

We accept referrals from professionals and from participants and families directly.

Support Coordinators

Refer participants preparing for plan reviews, SIL, SDA, ILO or STA pathways, or documenting complex support needs.

Health professionals

GPs, allied health and treating teams can refer where OT-led functional evidence is needed. We collaborate throughout.

Participants & families

You can refer yourself or a family member directly — no professional referral is required to get started.

A clinician-led assessment using standardised tools, with collaboration where required.

In most cases, reports can be issued within 7 days once interviews and documentation are complete.

How assessments are funded

Assessments are generally claimed under Capacity Building supports where funding is available in the participant's plan.

NDIA managed services are delivered through a registered provider entity, and we work directly with plan managers for plan managed participants.

There is no upfront payment required for NDIS funded assessments.

What information do I need to submit a referral?
Participant contact details, funding type (NDIA, plan or self managed), the assessment need, and any relevant reports you can share. If anything is missing, submit what you have — we follow up during intake.
How is the assessment funded?
Assessments are generally claimed under Capacity Building supports where funding is available. There is no upfront payment required for NDIS funded assessments.
